In-depth case review
What is an in-depth case review?
In-depth case review is a detailed and thorough investigation following an adverse event. It is less resource intense than the Root cause analysis (RCA) methodology.
When should an in-depth case review be conducted?
The methodology:
- should be used for a valid clinical incident with a severity rating (ISR) 2 in the Victorian health incident management system (VHIMS)
- may also be used to review incidents with an ISR 3 or 4.
Purpose of an in-depth case review
The in-depth case review process is used to review adverse events to assist in identifying process and system issues. The process should identify what happened, why it occurred, and what can be done to prevent it from happening again.
When should in-depth case review methodology not be used?
The methodology is not used for performance management related issues or any incident that is subject to review under the state or Commonwealth criminal justice system.

Choose from one of the following techniques to undertake the investigation (alphabetical order):
Technique 1 – Change analysis template
Technique 2 – Hazard barrier target analysis template
On occasions it may be appropriate to use two or more of the techniques to assist with the investigation.
All templates may be modified to suit local requirements.
