How to request data
The following steps outline the process to request hospital data (HOSdata) reports:

Review accessible and restricted data field lists
All applicants should review the accessible and restricted data field lists related to the collection of interest:
Victorian Emergency Minimum Dataset (VEMD) - Accessible & Restricted Data Fields
Victorian Admitted Episodes Dataset (VAED) - Accessible & Restricted Data Fields- Elective Surgery Accessible and Restricted Fields List - Accessible & Restricted Data Fields (available upon request).

If you require other data
If the accessible data and restricted data field lists do not fulfil your current data request, please contact the HOSdata front desk detailing the data fields you do require. When your request has been received you will be contacted by the HOSdata front desk to verify and further action the request.

An application form has been developed to provide structure and capture as many details as possible about the request.

HOSdata Application Form
This is an MS Excel workbook (Excel 2003 format) with three worksheets that need to be completed. The form collects information about the requestor, the request and a list of the required data fields.

If you require assistance completing any part of the form, please contact the HOSdata front desk.
Notes:
All requests must be made using the HOSdata application form unless otherwise specified. In order to ensure a responsive and efficient turn-around time please ensure:
- the application form must be fully completed
- required data fields are selected using the check boxes in the application form
- applications must contain enough information to enable a feasibility assessment
- please ensure all contact information is correct so we can contact you.
Submit the completed form
Submit fully completed application forms to the HOSdata front desk for action.

The HOSdata front desk will review applications and provide feedback.
A ‘Conditions of Release’ document will also be issued to the applicant for consent and to confirm release of the requested data.
All HOSdata requests will be based on the past financial year's data except for applications received that have substantial justification to receive current year’s data (if the data is available).
