Subject: Updated National Private Patient Claim Form

Purpose: To advise public hospitals of the new updated National Private Patient Claim Form

Mr Russell Schneider, Chief Executive Officer of Australian Health Insurance Association (AHIA), wrote in December 2004 to all Australian hospitals to advise of the introduction of the new updated claim form for use from 1 st March 2005.

The new updated National Claim form requires the provision of complete and accurate data on the claim form, which is submitted to private health insurance funds.

AHIA have now advised DHS that as from 1 st June 2005, claims received that do not use the new claim form or do not have all data elements completed will be returned to the respective hospital unpaid, for resubmission.
