What's changed with forms - Malitha Perera Principal Clinical Advisor

We’ve made minimal changes to forms.

The new forms follow the same structure as the Mental Health Act 2014 forms with updates to ensure they are compliant with the new Act.

…and information is still recorded in CMI.

The new forms have been distributed to Health Information Managers and Act Implementation Leads in designated mental health services.

There is one new form – MHWA-143. This is the Authority for Chemical Restraint.

This form will be supported by the Chief Psychiatrist’s guidelines regarding the regulation of chemical restraint.
