healthcare redesign programs. As well as measurable results, such projects provide a vehicle for a healthcare provider to learn and adapt proven improvement methods to their requirements. This can take place while the critical and more far-reaching leadership and cultural change elements are developed
This presentation on redesigning the patient journey formed part of the 'Change management and lean thinking in the health environment' forum held in April 2009. The forum was facilitated by the Australian Continuous Improvement Group. The day focused on the practical application of the lean thinking approach in the health environment, more specifically in the implementation of integrated chronic disease management. This summary acts as a companion document to the presentation A Lean thinking approach to redesigning care.
