Secondary school vaccine program information and consent forms

Consent forms for parents to authorise their child to be vaccinated.

All secondary school-aged students in Year 7 and Year 10 (or age equivalent) are offered vaccines. Young people under 18 need parental consent for vaccination unless they are considered a 'mature minor'.
