Each quarter, Victorian community health providers must report data on their funded services to us.

They must follow our guidelines on what they report and how frequently as outlined in the submission guidelines below.

Service providers send their data submissions via the Community Health Minimum Data Set (CHMDS) to us on a quarterly basis.

We then compile quarterly performance reports from the data sent via the CHMDS, and then post the reports to the Funded Agency ChannelExternal Link .

The quarterly data submissions due dates are:

  • Quarter 1 - July to September: due 15 October
  • Quarter 2 - October to December: due 15 January
  • Quarter 3 - January to March: due 15 April
  • Quarter 4 - April to June: due 15 July
