  • Inadequate hydration
  • Inability to request assistance
  • Lack of recognition of early urges to go
  • Inability to remember what is wanted
  • Inability to recognise a toilet
  • Poor visibility of toilets
  • Poor access to toilets
  • Mobility restrictions
  • Inadequate history or charting of a person
  • Medications such as anti-cholinergics, sedatives, diuretics and atropinic drugs
