Planning the future of Victoria’s sub-acute service system: a capability and access planning framework - February 2013 outlines a comprehensive strategy to ensure fair access to high-quality, integrated subacute services across Victoria.
Vision
Victoria’s vision is for a modern, integrated subacute services system aimed to better meet the complex health care needs and expectations of individuals and the community.
A key focus of program and policy development includes better alignment and integration of community-based programs to support the discharge from admitted services and to prevent or substitute hospitalisation.
Aim
The subacute planning framework will develop an evidence-based approach to guide planning towards equity of access and consistency of service quality.
Key features of framework
Key features of the subacute services capability framework are that it:
- develops explicit expectations about service standards, scope of practice and resources required to deliver subacute services at designated level
- allows an expected subacute service profile to be developed to aid planning at metropolitan, regional and statewide levels
- develops statewide benchmarks to measure access to subacute services currently, and into the future.
