Diabetes Connect

Find out about Diabetes Connect, a community health-led integrated care pathway for people with type 2 diabetes.

Diabetes Connect is a community health-led integrated care pathway for people with type 2 diabetes. It is being piloted by select community health services across Victoria until June 2025.

Diabetes Connect seeks to improve health and wellbeing outcomes for people with type 2 diabetes, reduce avoidable diabetes progression and hospitalisation, and improve coordination between primary, community and acute care services.

Diabetes Connect will provide:

  • a care coordination approach that connects people with the clinical and social services and supports they need to meet their health and wellbeing goals, with a focus on social prescribing and improving self-management capacity
  • risk adjusted interventions including health coaching, remote monitoring and virtual check-ins, with response and escalation protocols to manage clinical and or social deterioration
  • a flexible and risk adjusted funding model to promote better care coordination and workforce flexibility, and improve access to multidisciplinary care for the management of chronic disease.

Diabetes Connect pilot sites

Diabetes Connect will be piloted in five locations across metropolitan and regional Victoria and will run until 30 June 2025. The pilot sites are:

Diabetes Connect is being funded as part of the Commonwealth Government’s Primary Care Pilots initiative which tests innovative models that reduce pressure on Emergency Departments.
